How it works

Get in touch
This is where it all starts - reach out to us in the way that suits you best. This could be on the phone, dropping into one of our hubs, via our online referral form, speaking to your GP or another professional.
We'll be able to provide you with an appointment with one of our experts in whichever service best meets your needs, or we'll give you a call back to offer you an appointment. All the ways you can get in contact can be found here. -
Your first session
This is where you'll meet your Horizons worker and the opportunity to talk about your story and discuss the support you might need. You can share as much or as little as you like, at your own pace.

Our levels of support
Your Horizons worker will discuss the various forms of support that are available to you. From advice and information, prescribed medication, detox programs, one-on-one support, or participation in a group of people with similar experiences, we'll help you explore all your options.

Plan your programme
Together, with your Horizons worker, we'll agree a programme that feels manageable, achievable and entirely based on your choices. This could include activities to enhance your wellbeing, or if you need help with housing or job searches, we can support with that as well.
Our approach to supporting you
Everyone is unique, so we believe in being flexible. You can choose the approach that works best for you, starting with small steps—no change is too big or too small. If you prefer not to make changes right now, that's perfectly fine too. We can assist you in staying safe, reducing use, or stopping altogether.
